Application Instructions

Please read the instructions below before beginning your online application. Failure to follow instructions may result in a delay in processing your application. Incomplete applications will not be considered. If an application remains incomplete for more than 8 weeks, it will be discarded from the online system and you will need to begin a new application.

Before You Begin

  1. Review the complete CACP Candidate Handbook to verify that you meet the eligibility requirements and to review the application requirements.

  2. Select an approved exam site and know your first, second, and third choice exam date.  The number of examinees assigned to each exam date is limited to optimize system performance and esure short wait times in the event that phone support is needed.  Once approved, you may reschedule your exam for any available exam date within 12 months of your initial approval date (retake examinees excluded). Although you may take your exam at any approved site, the NCBAP does not have prior arrangements made with each site. It is your responsibility to contact your desired exam site and verify that the site (1) can accommodate an online exam at a PC with Internet Explorer 6.0 or higher and (2) can proctor your 2.5 hour exam on the date(s) and time(s) you are considering. Upon approval of your application, the NCBAP will send an email to your exam site with additional details about proctoring your exam.

     
  3. Prepare your written description of current activities pertaining to anticoagulation therapy management. Include your full name and date of birth at the top of the page. As described in the Candidate Handbook, the written description should be 500 words or fewer and should describe your general practice and experience specifically in antithrombotic therapy management. Duration and types of activities should be described, including number of patients, practice setting, management of clinic, your roles and responsibilities, and coworkers, etc. Paragraph format should be used; a resume or curriculum vitae will not suffice. When completing the online application, you will be asked to upload your practice description as either an MS Word (.doc) or pdf document. No other file types will be accepted. If your MS Word document was created in Word 2007 (i.e. has .docx extension), please save it with a .doc extension before uploading.

    Important Note for Retakes: If you initially took the exam prior to December 31, 2009 and received a letter indicating that you are eligible to retake the exam for a discounted fee if taken within 12 months, then in lieu of a written practice description, you should upload a letter indicating your intent to re-examine and note the date of your first exam. Please include your full name and date of birth at the top of the page.

  4. If you have a documented disability or qualifying temporary or permanent medical condition and would like to request special accommodations for the CACP exam in accordance with the Americans with Disabilities Act, prepare your written request before beginning the online application. See "Special Accommodations" below or refer to the CACP Candidate Handbook for a description of what must be included in your request. When completing the online application, you will be asked to upload your request as either an MS Word (.doc) or pdf document. No other file types will be accepted. If your MS Word document was created in Word 2007 (i.e. has .docx extension), please save it with a .doc extension before uploading.

  5. Have your credit or debit card ready so that you can remit payment upon completion of the application.

For Faster Processing

If you have access to a scanner, you can speed up the processing of your application by scanning the following documents before beginning your application:

  1. Current US professional license
  2. Current government-issued photo ID
  3. Signature statement provided in the CACP Candidate Handbook

You have the option of sending these 3 documents by fax or email after completing the online application. However, you can speed up the processing of your application by uploading a copy of each document when completing your online application. If you have access to a scanner, please scan a copy of each document prior to beginning your application and save the scanned document to your hard drive. When completing the online application, you will be given an opportunity to upload each of these documents. The following file types are allowed: .pdf, .jpg, .gif, .tiff, .bmp

Exam Retakes & Discounts

If you have previously received a letter from the NCBAP indicating that you are eligible to take the exam at a discounted rate, you must contact Marie Walker at info@ncbap.org to request a discount code before beginning your online application. You will need to provide this discount code on the payment screen upon completion of the online application.

Special Accommodations

The NCBAP complies with the Americans with Disabilities Act and provides reasonable and appropriate accommodations for those with documented disabilities and for those with a qualifying medical condition that may be temporary. At least two exam dates per year will be scheduled at which the NCBAP will provide for special accommodations.

If you wish to request special accommodations, you must provide a written request when completing your online application. Your request must include:

  1. A description of the special accommodations you would like the NCBAP to provide you
  2. A description of your disability or medical condition
  3. Details of any special accommodations you have been granted on exams adminstered by other examination boards

When completing the online application, you will be asked to upload your request in either MS Word (.doc) or pdf format. No other file types will be accepted. Please include your full name and date of birth at the top of the page, and save your written request to your hard drive before beginning the online application.
